The information is accurate to the best of your knowledge
You have the right to share it
It does not include confidential detainee/patient information or PHI

You grant us the right to use your submission to respond to you and operate our business communications.

The Site may rely on third-party platforms (e.g., website hosting, scheduling, analytics) or include links to third-party sites. We are not responsible for third-party services or content.

8) Disclaimers

TO THE MAXIMUM EXTENT PERMITTED BY LAW, PULSE PATCH WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR ANY LOSS OF PROFITS, REVENUE, DATA, OR GOODWILL, ARISING OUT OF OR RELATED TO YOUR USE OF THE SITE.

IN NO EVENT WILL OUR TOTAL LIABILITY EXCEED ONE HUNDRED U.S. DOLLARS (US $100) OR THE AMOUNT YOU PAID TO USE THE SITE (IF ANY), WHICHEVER IS GREATER.

You agree to defend, indemnify, and hold harmless Pulse Patch and its officers, directors, employees, and agents from any claims, liabilities, damages, losses, and expenses (including reasonable attorneys’ fees) arising from your use of the Site or violation of these Terms.

11) Termination

We may suspend or terminate access to the Site at any time, with or without notice, if we believe you violated these Terms or if necessary to protect the Site and users.

12) Governing Law and Venue

These Terms are governed by the laws of California without regard to conflict-of-laws rules. Any dispute arising from these Terms will be brought in the courts located in California and you consent to their jurisdiction.

13) Changes to These Terms

We may update these Terms from time to time. The “Effective Date” will reflect the latest version. Continued use of the Site after updates means you accept the revised Terms.
